Focal epilepsy without interictal spikes on scalp EEG: A common finding of uncertain significance
Epilepsy Research, 150, 1-6. DOI 10.1016/j.eplepsyres.2018.12.009
In brief
Doctors use interictal spikes on EEG to locate and classify epilepsy, but not every patient shows them. Reviewing seven years of video-EEG monitoring at the Calgary Epilepsy Program, this study measured how often adults with focal epilepsy never spike on scalp EEG and whether they differ clinically from those who do.
Key findings
- 933 monitoring admissions reviewed; 345 patients met the criteria
- 17% showed no spikes despite prolonged monitoring
- No clinical differences found, suggesting smaller or deeper seizure generators the scalp cannot see
